Evolving Practice of Nursing and Patient Care Delivery Models

The health care delivery system will experience a change in due time and this abounds from the government’s efforts to restructure the system in order to improve the delivery of service and making it accessible to many people in the country. Thus, it is essential to be aware of the changes that will take place in the delivery of services, as it will involve the shift of nursing jobs from acute care hospitals to community based. The government is focusing on a few areas to facilitate restructuring of the health care system. The areas that the government focuses on include the introduction of nurse-managed health clinics, continuum of care, accountable care organizations and medical homes.

According to American Nurses Association, the government is promoting the development of nursing practice arrangements, as this will cut insurance costs. It is for a reason that it is predicated that the primary care givers will not be in a position to meet the increase in demand. This is a clear proof of the shift in nursing jobs because nurse practitioners will supervise these nursing arrangements or nurse-managed health clinics.

Nurses are also expected to be familiar with the continuum of care. It is for a reason that the government plans to focus its finances on expansion of the continuum of care to facilitate the primary care providers to meet the rise in demand of health services. It is also essential for nurses to know the significance of health care follow up. This is done after a patient is discharged from the hospital and a nurse can provide services to the patient at their residence. This is a shift of nursing services from the acute care hospitals to the community.

In an effort to support the government’s efforts of facilitating the provision of health care services, health care providers have come up with Accountable Care Organization (ACO). Besides, ACO are focused on the provision of care to a group of patients. Additionally, it should be noted that ACOs are formed by a group of professionals who are willing to be accountable for the cost, quality and care of Medicare beneficiaries enrolled to their services. This also proves a shift in nursing services from acute care hospitals to the community as nurses will be required to offer their services in such places.

Medical Homes have been in existence since the 1980s in some countries. However, United States adopted the system recently in 2002 when the first Medical Home was established. The government considers Medical Homes as one way of assisting primary care providers to meet the projected future demand for health care services. Physicians manage Medical Homes and they provide a wide range of health care services. This explains how the nursing practice will shift and it is critical for all nurses to be conversant how Medical Homes operate.

In conclusion, there is a projected shift of nurse services from acute care hospitals to the community. This abounds from the government’s efforts to ensure that the primary health care providers can meet the increasing demand for health care. Thus, principal points relating to how the shift will be effected to the community include promotion of Medical Homes, use of Nurse-managed health clinics, Accountable Care organizations and the continuum of care.
